Beneficial effect of midodrine in hypotensive cirrhotic patients with refractory ascites.

چکیده
Ascites is the most frequent complication of cirrhosis, occurring in nearly 50% of patients within 10 years after cirrhosis is diagnosed.1 A proportion of these patients require large-volume paracentesis (LVP) for symptom relief when other treatment modalities are unsuccessful or impossible. However, frequent LVP is associated with patient discomfort, decreased quality of life, and increased potential risks, such as circulatory dysfunction and related complications.2 Vasoconstrictors are known to have beneficial effects in patients with hepatorenal syndrome (HRS), as these agents can improve renal disease related to circulatory dysfunction.3-5 We report 2 patients requiring frequent LVP who benefited from midodrine treatment, as evidenced by the decreased volume of ascitic fluid drained; midodrine treatment also reduced the frequency of paracentesis and, thus, its potential risks.

Midodrine in patients with cirrhosis and refractory or recurrent ascites: a randomized pilot study.
BACKGROUND & AIMS Splanchnic arterial vasodilatation plays an important role in cirrhotic ascites. The aim of this study was to evaluate the effects of long term administration of midodrine on systemic hemodynamics, renal function, and control of ascites in patients with cirrhosis and refractory or recurrent ascites. METHODS Forty cirrhotic patients with refractory or recurrent ascites were p...
متن کاملSystemic hemodynamic and renal effects of midodrine and octreotide in cirrhotic patients with ascites
e read with great interest the article by Minakari et al. regarding the effects of treatment with subcutaneous octreotide and oral midodrine in cirrhotic patients with ascites who continued to receive their standard diuretic treatment1 and we would like to comment on their results. First, the administration of midodrine for 3 days caused a non-significant increase in mean arterial pressure (MAP...
متن کاملClinical study on the therapeutic role of midodrine in non azotemic cirrhotic patients with tense ascites: a double-blind, placebo-controlled, randomized trial.
BACKGROUND Midodrine is an α-agonist prodrug of desglymidodrine used for the management of hypotension. Midodrine has demonstrated usefulness in hepatorenal syndrome. OBJECTIVE The objective of the present work was to study the role of midodrine in patients with non-azotemic cirrhosis with tense ascites. METHODS This prospective randomized double blind placebo-controlled study was conducted...
متن کاملComparison of the effect of midodrine versus octreotide on hemodynamic status in cirrhotic patients with ascites
BACKGROUND In cirrhotic patients peripheral vasodilatation may decrease renal blood flow and subsequently raises plasma renin activity. Octreotide with several mechanisms causes peripheral arterial vasoconstriction. Midodrine is an alpha agonist and acts as a peripheral vasoconstrictor; therefore it may reduce plasma renin activity and improve renal function. In this study the effects of these ...

Received: 16 Oct, 2012 Accepted: 22 Dec, 2012 Background & Aims : Liver cirrhosis is associated with coagulation disorders due to decreased synthesis of clotting and inhibitor factors. It has been hypothesized that ascites plays an important role in hyperfibrinolytic state and increased risk of bleeding. We have studied the role of ascites in the hyper fibrinolytic state and relationsh...
